Periodontal implants are endosseous dental implants placed and maintained within the context of the periodontal and peri-implant tissues, used to replace missing teeth in patients who often present with prior periodontal disease or alveolar bone loss. Unlike a natural tooth held by a periodontal ligament, an osseointegrated implant bonds directly to bone, so its long-term success depends on the quality and quantity of supporting bone, the health of the surrounding soft tissue, and control of peri-implant inflammation. Achieving a stable result frequently requires reconstructive and site-preparation procedures, including ridge and sinus augmentation with bioactive graft materials such as calcium phosphosilicate, the socket shield technique to preserve the buccal bone plate during immediate placement, and the use of titanium mesh and growth factors to rebuild severely atrophic ridges. Prosthetic considerations cover impression techniques for closely placed implants, occlusal contact assessment for implant restorations, and the choice of conventional versus corticobasal implant designs when standard two-stage implants fail. Skeletal anchorage applications, including mini-implant–supported appliances, also draw on the same biological principles. Collectively the field addresses how implant geometry, surgical protocol, bone management, and occlusal loading interact to restore function and stability in periodontally compromised dentitions while minimising the risk of peri-implant breakdown.
